From Evidence to Action: A 5-Year Roadmap for Community Health Transformation in Kenya
The Utafiti Wellness Strategic Plan 2026-2031 represents our bold commitment to transforming community health in Kenya through evidence-based innovation. This comprehensive roadmap outlines our vision to bridge the gap between health research and community implementation—a gap that has historically left life-saving discoveries confined to academic journals rather than reaching the communities that need them most.
Over the next five years, we will focus on scaling our impact, strengthening partnerships, and pioneering innovative approaches to health and wellness. Our strategy is built on five interconnected pillars: Research Excellence, Community Empowerment, Innovation & Digital Health, Strategic Partnerships, and Organizational Sustainability. Each pillar represents a critical dimension of our work, and together they form an integrated framework for systemic change.
By 2031, Utafiti Wellness aims to have directly impacted over 100,000 community members, trained 500+ community health champions, published 15+ peer-reviewed research articles, and established sustainable partnerships with at least 10 county governments. This plan is not merely aspirational—it is a concrete, actionable roadmap grounded in Kenya's health priorities and global best practices in research translation.
If we translate rigorous research into accessible, culturally-adapted interventions AND equip community champions with evidence-based tools AND foster sustainable partnerships with health systems, THEN communities will experience measurable improvements in health outcomes, AND Kenya will move closer to achieving Universal Health Coverage.
Understanding the landscape that shapes our strategic priorities
Kenya faces a dual burden of communicable and non-communicable diseases. Maternal mortality stands at 355 deaths per 100,000 live births, while NCDs account for over 50% of hospital admissions. The gap between research evidence and implementation remains wide—studies show it takes an average of 17 years for research findings to reach practice. Utafiti Wellness exists to collapse this timeline.
Our strategy directly supports Kenya Vision 2030, the Kenya Health Sector Strategic Plan (KHSSP), and Universal Health Coverage (UHC) goals. By focusing on research translation, we contribute to the government's priority of strengthening health systems through evidence-based decision-making and community-centered interventions.
Our work aligns with Sustainable Development Goal 3 (Good Health and Wellbeing), the WHO's Global Strategy on Human Resources for Health, and the Astana Declaration on Primary Health Care. We position Utafiti Wellness as a contributor to global learning on research translation in low- and middle-income country contexts.
Baseline assessments conducted in 2025 across 5 counties revealed critical gaps: low awareness of preventive health measures, limited access to translated health information, and a strong desire among community members to be active participants in health solutions rather than passive recipients of aid.
Five interconnected pillars that form the foundation of our 5-year strategy, each with specific goals, initiatives, and measurable outcomes
Conducting cutting-edge, participatory research that identifies community health priorities and generates actionable evidence. This pillar emphasizes co-design with communities, ethical research practices, and rapid translation of findings into practice.
Equipping communities with knowledge, skills, and resources to become active participants in their health journey. This includes training Community Health Promoters, establishing peer support networks, and fostering health literacy through culturally appropriate channels.
Developing and scaling innovative health technologies and digital solutions that enhance healthcare access and quality. This includes mobile health applications, telehealth platforms, and low-cost diagnostic tools designed for low-resource settings.
Building robust networks with government ministries, academic institutions, civil society organizations, and private sector actors to amplify our impact and ensure sustainability. Partnership models range from formal MOUs to collaborative research agreements.
Building a strong, resilient organization with diversified funding streams, professional staff capacity, robust governance structures, and systems for monitoring, evaluation, and learning.
Our phased approach to achieving transformational impact, with clear milestones and accountability structures
How we track progress, measure impact, and continuously improve
Proactively identifying and addressing potential challenges
Mitigation: Diversify funding sources (grants, individual donors, corporate partnerships, earned income). Maintain 6-month operating reserve. Develop endowment fund by Year 5.
Mitigation: Competitive compensation, professional development opportunities, succession planning, documentation of institutional knowledge.
Mitigation: Maintain relationships across political spectrum, focus on evidence-based advocacy, diversify geographic presence, remain non-partisan.
Mitigation: Phased approach with clear milestones, buffer time built into timelines, adaptive management, regular progress reviews.
Together, we can transform community health in Kenya. Whether you are a researcher, community leader, donor, or partner organization, your engagement can help turn evidence into action.
